Voter Registration Forms in PDF format Created as an aide to musicians who wish to provide voter registration forms at their performances, here is an archive of voter registration forms for each state in PDF format. Download the appropriate form for the state(s) you'll be performing in, and make copies in advance. Encourage your audiences to vote, and make it easy for them to register by making forms available at your performances. Please be aware that voter registration in some states has special requirements and exceptions, and each PDF document contains a list of these on its final page. New Hampshire town and city clerks will accept the national application only as a request for their own absentee voter mail-in registration form. North Dakota does not have voter registration. Wisconsin town, village and city clerks will accept the national application only as a request for their own voter mail-in registration form. Wyoming by law, cannot accept the national form unless state law is changed.
